OKARCHE, Okla. A former Okarche police chief has been arrested after an Oklahoma State Bureau of Investigation probe concluded he allegedly used another person's notary signature and stamp without permission during a city audit.

The investigation began on Feb. 14, 2025, when the Canadian County District Attorney's Office requested the Oklahoma State Bureau of Investigation investigate allegations of fraud involving former Okarche Police Chief Forrest Smith, 52.

Smith served as the town's police chief from 2018 until 2024.

According to the OSBI, the investigation centered on an affidavit Smith signed during a private audit of the City of Okarche in April 2024.

Investigators said Smith notarized an affidavit stating he had complied with the audit process. However, the document contained the notary signature and official stamp of a police department supervisor who was not at work on the day the affidavit was executed.

The OSBI determined during its investigation that Smith allegedly used the supervisor's signature and notary stamp without authorization.

Based on the evidence gathered, the Canadian County District Attorney's Office obtained an arrest warrant for Smith on July 29, 2026, charging him with Impersonating Another in Execution of Instrument.

Smith turned himself in to the Canadian County Detention Center on July 30, where he was booked into jail.

The Oklahoma State Bureau of Investigation thanked the Canadian County District Attorney's Office for its assistance throughout the investigation.

The investigation remains ongoing.
